Official Rules for the 2025 Streets to Creeks Creek QR Code Contest

The 2025 Streets to Creeks Creek “Golden” QR Contest is sponsored by the City of Santa Rosa, Town of Windsor, City of Healdsburg, City of Cloverdale, City of Ukiah, City of Sebastopol, and County of Sonoma (“Sponsors”). No fee, purchase or subscription is necessary to enter or win the 2025 Streets to Creeks Creek QR Code Contest Drawing (“Drawing”). This Drawing is subject to the complete Official Rules listed herein.

1. Eligibility:

1. To be eligible to enter or win the Drawing you must be a person that is at least 21 years of age and a resident of Sonoma or Mendocino Counties. 

2. Employees, directors and officers of the Sponsors and their immediate family members, and/or members of their households, are not eligible to participate in and/or win the Drawing.

3. Entrants must have a device capable of scanning QR codes and access to the internet to fill out the submission form. Entries must be submitted through the submission forms located on the Streets to Creeks website to be considered.

4. By participating in this Drawing, you agree that:

1. The information you submit through Drawing's form will be shared with the Sponsors and may be used to contact you;

2. Drawing Deadline:

   The deadline to enter the Drawing is July 31, 2025, 11:59:59 p.m. (PDT).

3. Prizes:

   One (1) winner will be randomly selected to receive one (1) grand prize. Prize is non-negotiable and cannot be exchanged for a cash equivalent.

4. Odds of Winning:

   Odds of winning will be determined by the number of eligible entries received.

5. How to Participate in the Drawing:

1. Entry procedures: One QR code, known as the “golden” QR code, in each of the trash pilot areas (within the City of Santa Rosa, Town of Windsor, City of Healdsburg, City of Cloverdale, City of Ukiah, and City of Guerneville) will be replaced with a QR code that takes the scanner to the webpage with the Drawing entry form. Scan the QR codes located on Streets to Creeks creek critter stickers or signs installed in one of the trash campaign areas until one of the “golden” QR codes is found and scanned. Entrants must submit first name, last name, and contact information of the entrant to be entered in the Drawing. Please note: Any entries containing inappropriate, vulgar, or illegal content will be void and disqualified from participation. Duplicate entries will not be eligible to participate in and/or win the Drawing.

2. The Sponsors are not responsible for error, omission, interruption, deletion, defect, delay in operations or transmission, loss, theft or destruction or unauthorized access to or alterations of entry materials, or for technical network, telephone equipment, electronic, computer, hardware or software malfunctions of any kind, or inaccurate transmission of or failure to receive entry information from the Sponsors on account of technical problems or traffic congestion on the Internet, at any website or any combination thereof.

3. Your entry will be disqualified, at the discretion of the Sponsors, if you (or if you attempt) to (i) enter the Drawing through any means other than as described in these Official Rules; or (ii) to disrupt the Drawing, circumvent the terms and conditions of these Official Rules, or in any way tamper with the Drawing. If either of the above occurs, the Sponsors have the right to remedy any such action, disruption, or circumvention, and to seek damages from you to the fullest extent permitted by law.

6. Selection of the Winner:

1. After the contest deadline on July 31, 2025, at 11:59 PM PDT, all eligible entries will be compiled, and the winner will be selected through a random drawing conducted by the contest sponsor. The drawing process ensures that every valid entry is considered equally.

2. Within 14 days of notification attempt, the winner will be required to sign and return the Waiver and Release form found at the bottom of this page.

   The winner may be required to sign and return additional applicable documents sent by Sponsors related to the Drawing which may include but are not limited to, a letter of prize acceptance, and/or an affidavit of eligibility, within 14 days of notification attempt. Failure to return any requested documents in the time noted will result in disqualification of the entry and the random selection of an alternate winner.

3. If the winner does not comply with the Official Rules of the Drawing, he or she will be disqualified and an alternate winner will be selected.

4. Return of any prize or prize notification as “undeliverable” will result in disqualification and an alternate winner will be selected.

7. Notification:

   The winner will be notified by emailing the address and calling the phone number submitted to the Sponsors. The Sponsors are not responsible for notifications that are misdirected or not received. If a potential winner cannot be reached after a reasonable effort has been made during 10 business days from the first notification attempt, such person may be disqualified, and an alternate winner may be selected. Any prize notification or prize returned as undeliverable may result in the award of the prize to an alternate winner. The prize is non-transferable and no substitutions or cash equivalents are allowed. The Sponsors reserve the right to award a prize of equal or greater value if the advertised prize is unavailable.

8. Conditions:

1. By participating in the Contest or Drawing, you agree to release and hold the Sponsors and its directors, officers, employees and agents harmless from any and all injuries, losses, damages, rights, claims, demands, liabilities, expenses and actions of any kind in connection with participating in the Contest; Drawing; or resulting from acceptance, possession, use or misuse of any prize, including, without limitation, personal injuries, death, and property damage, and claims based on publicity rights, defamation, or invasion of privacy, whether suffered by you or a third party. Sponsors make no warranty, representation or guarantee, express or implied, regarding the prizes, including without limitation, quality, merchantability, or fitness for a particular purpose.

2. If you win a prize, each person who wishes to occupy or use the rental property as part of that prize must execute the Waiver and Release referenced in Section 6(b) prior to entering or using the property.

3. The Drawing is void wherever prohibited or restricted by law and are subject to all applicable federal, state and local laws and regulations. By participating in the Drawing, you also agree to be bound by these Official Rules and the decisions of the Sponsors and waive any right to claim ambiguity in the Drawing or these Official Rules. All decisions regarding the Drawing, including, but not limited to, selecting the winner, will be made by the Sponsors, whose decisions shall be final.

4. The winner is solely responsible for all applicable federal, state and local taxes associated with winning the prize. Winners should consult a tax professional to understand their obligations and plan accordingly.

9. Choice of Law:

   The Drawing shall be governed by, construed and enforced in accordance with the laws of the State of California. Any action regarding the Drawing shall be brought in the County of Sonoma. Drawing rules are subject to change at any time without notice.
